Covered Under a 90/10 Coinsurance Plan
Plan Pays 90% and the Participant Pays 10% up to the $2,500 Annual Family Allowance
Annual Family Allowance of $2,500
This is the Family Annual Allowance per benefit year (May 1 through April 30). Once the annual allowance is met, dental services are paid 100% by the Participant for the remainder of the benefit year
Preventative & Diagnostic Care Covered 100%
$0 Coinsurance for Preventative and Diagnostic care. However, the claims costs paid by Cigna will go against your $2,500 annual allowance.
$3,200 Lifetime Orthodontia Allowance
$3,200 Lifetime Orthodontia Benefit per eligible family member. This allowance is separate from the Dental Annual Family Allowance

Eye Exam Covered 100%
One per year for each eligible family member ($40 allowance for out-of-network providers).
Allowance of $100 for Frames, Lenses, or Contacts
Once per year for each eligible family member (same allowance for out-of-network).
Additional $50 Allowance at Visionworks
A total allowance of $150 for frames, lenses, or contacts at Visionworks providers (In-network only)
Once Per Year For All Eligible Participants
All eligible Participants can now go for exams, lenses, frames, or contacts each Benefit Year (May 1 through April 30).
